Hi Zohra. As the kittens are already in 5th week, every 4-5 hours for 20- 30 min would suffice. Formula milk can also be started as they are approaching weaning. It is natural for a mother cat to lose interest in nursing kittens once they are a month old.

For formula milk (should we completely boil the water and then cool it or luke warm water is fine )

You should boil it, cool it down and use a Luke warm one.
