I have used some powder prescribed by doctor which did not make any difference also shampoos

Ok. You can prefer using a Spot on ( which should be selected based on body weight, use Nayflee or Fiproforte plus spot on ). This comes with a caution to avoid licking. So should make sure a lucky doesn't lick the body atleast 4 hours post application. You can repeat after 4/5 months. Other option could be a tablet. You can use Tab Simparica or Bravecto if spot on is not preferred.

It could be due to phosphorus and calcium deficiency. But the left leg looks more bent. I suggest you to visit a vet and get clinical examination along blood work plus x ray done.
